Consumer Health Data Privacy Policy

This policy explains how Huddle Men's Health handles consumer health data of Washington residents collected through huddleupmenshealth.com, including our Bellevue pages, under Washington's My Health My Data Act.

1. What we collect and why

• Contact details you enter: your first name, mobile number and email. We use them to reply to you, schedule your free visit and send the messages you agreed to.
• The fact that you asked for a TRT consultation or free blood work. This can say something about your health. We use it to schedule and prepare your visit.
• Appointment details: the time you book, and whether you attend and start care. We use them to run the clinic.
• Ad click identifiers (such as gclid, gbraid and wbraid) and the page you arrived on, when you come to us from an ad. We use them to know which ad brought you to us and, only if you agree, to measure our ads with Google.

We do not ask about symptoms, conditions or medications on this website.

2. Where we get it

• From you, through our forms and our booking calendar.
• From your browser, when you click one of our ads (the click identifier is part of the web address).

3. What we share, and with whom

• Contact and appointment details, with the service providers that run our systems for us: our customer relationship and scheduling system, and our text message and email providers. They use the data only to provide those services to us.
• The ad click identifier and whether you booked, attended or started care, with Google (advertising), only if you tick the optional box that lets us share that you responded to an ad. We never share your name, email, phone number or health information with Google.

We do not sell consumer health data.

4. Your rights

You can:

• Confirm whether we collect, share or sell your consumer health data, see that data, and get a list of everyone we shared it with.
• Withdraw your consent to sharing at any time.
• Have your consumer health data deleted.

To use these rights, call our Bellevue clinic at (425) 233-8623, or write to Huddle Men's Health, 1515 116th Ave NE, Suite 108, Bellevue, WA 98004. We answer within 45 days. If we need more time, we may extend this once by another 45 days and will tell you why. Requests are free, up to twice a year.

If we decline your request, you can appeal by replying to our decision through the same contacts. We answer appeals within 45 days. If you disagree with the result, you can contact the Washington State Attorney General at https://www.atg.wa.gov/file-complaint.
